Nash UNC Health Care Foundation Receives $30,000 Contribution from PNC Bank to Sustain Impactful Community Paramedic Program
PNC Bank has contributed $30,000 to the Nash UNC Health Care Foundation in support of its Community Paramedic Program – an initiative that helps reduce hospital readmissions and connects local patients to various outpatient resources to support their health and well-being.
“We are grateful to PNC for this generous gift that will allow us to continue to grow this program and accomplish our goal to improve the health of our community,” said Foundation Director Kathleen Fleming. “The Community Paramedic Program provides at-home care for high-risk patients to reduce the likelihood of readmission to the hospital – and ultimately empowers patients to manage their health and minimize unnecessary emergency room visits.”
The Foundation has worked to expand the program since its launch in November 2020, growing from one paramedic to three and substantially increasing the number of patients the program can reach for follow-up care after discharge.
